Mission Statement
The Stevens Institute of Technology sports medicine office is committed to providing comprehensive, high-quality medical services to every student-athlete.  On-site medical services provided include, but are not limited to, prevention, evaluation and treatment of injuries and illnesses, rehabilitation services, pre-participation exams, nutritional counseling, psychological counseling and drug education/counseling.
The sports medicine team is comprised of one team physician, one orthopedic surgeon, one registered nurse, three licensed certified athletic trainers, and medical consultants.  This professional staff is responsible for the healthcare and safety of over 470 student-athletes participating in 25 Division III sports in the Empire 8 Conference.
 
Services
The Stevens sports medicine staff provides care to the Institute's student-athletes, including injuries, illnesses, rehabilitation and/or other personal issues affecting one’s physical or mental status.  In order to provide such comprehensive medical care, the sports medicine department maintains a board certified and specialty-trained team physician and a registered nurse.  The team physician will have a comprehensive referral source of other specialty-trained physicians, including orthopedic surgeons, neurologists, physiatrists, cardiologists, endocrinologists, gastroenterologists, podiatrists and mental health personnel.  A list of these referral services is kept in the sports medicine office.
 
The sports medicine office offers individual medical consultations on a weekly basis.  Please see our weekly office hours schedule for physician schedules.
 
Conduct
The athletic trainer at Stevens Institute of Technology is licensed by the State of New Jersey and therefore is regulated under the rules and regulations of the N.J.S.A 45:9-37.35.  The athletic trainer shall work “at the direction of a licensed physician."
 
Pre-Participation Examinations
NOTE - for all new athletes (freshman, transfers, new team members)
 
Stevens Institute of Technology requires that all athletes participating in NCAA varsity sports obtain a physical examination clearance from their family primary care physician prior to reporting for their season.  This includes any orthopedic injuries, cardiac conditions, head injuries and other serious medical conditions which may require treatment from the athletic training staff.  Pertinent medical information should be forwarded to the sports medicine office to help expedite an athlete’s care.  Athletes with special conditions need to be evaluated by the appropriate specialist if referred by our medical staff.  Please be advised that incomplete information regarding medical history or insurance coverage may result in delay of clearance; therefore, it is in your best interest to complete these forms carefully and without omission.
 
You may print out the Athletic Training Packet (revised copy coming soon) for your convenience.  Physicals are valid for two years from date of exam.
 
All athletic training packet forms must be returned directly to the athletic training office by August 1.  Athletes WILL NOT be permitted to participate in their sport in ANY capacity without such paperwork turned in and FULLY COMPLETED.
 
Insurance Coverage & Claims Processing
All intercollegiate athletes attending Stevens Institute of Technology are required to purchase the Student Health Insurance Plan through Consolidated Health Plans unless proof of comparable coverage is furnished.  It is highly recommended that all international students as well as non tri-state area students purchase this insurance policy to ensure timely medical care.   An outline of this coverage is available online at www.universityhealthplans.com.  Please review this information for details regarding your coverage.  Should you need to file a claim, please review their website for further information.
 
Stevens Athletics provides additional coverage for all varsity student athletes.  This policy is an excess policy with a $2,000 deductible and will pay the balance left from the primary insurance up to $75,000.  Medical expenses related to student-athlete's participation in VARSITY athletics only, that are not paid by your primary care insurance company may be submitted to AGIA, Inc.  This policy does not cover intramural or club sports.  For more information regarding the coverage of this plan, you may contact AGIA, Inc. directly at (800) 399-2560.
 
For claims relating to the AGIA, Inc. plan, please follow these guidelines:
1. All injuries must be reported to the athletic training staff immediately.  This will be documented appropriately by the ATC.
2.  The student-athlete must report to the athletic training room to fill out the appropriate claim forms to be submitted with the explanation of benefits and denial letter.
3.  All medical bills will be sent directly to the student-athlete’s parents or guardian.
4.  Parents should submit all bills to their primary insurance company for payment.
5.  For any medical bills NOT COVERED by your primary insurance company, you need to submit an explanation of benefits (EOB) and letter of denial to AGIA, Inc.
6.  Please include a copy of the itemized bill when you send your information to AGIA, Inc.
7.  Bills must be processed within 90 days of DATE OF INJURY.
8.  Do not send medical bills to the athletic training office.
 
Notice of Privacy Act
All student-athlete’s medical information is protected by HIPAA guidelines.  Stevens Institute of Technology includes a copy of this form to be signed by the student (or guardian if under 18 years of age).  The HIPAA form describes how their medical information may be used and whom may have permission to view it.
 
Visiting Team Information
The team physicians are not present but are on-call for all athletic events.  In addition, the following athletic training services will be provided:
1.  Use of our facilities for taping and treatments
2.  A certified athletic trainer will be provided for all on-campus events
3.  On-site EMS services for selected athletic competitions

The following supplies will be provided at home events:
1.  Water
2.  Cups
3.  Ice and ice bags
4.  Emergency equipment (crutches, splints and AED)

The training room will be open at least one and a half hours prior to the start of an event.  Water and supplies will be set up one hour prior to the start time.  If you need access to the training room earlier, please contact the athletic training office prior to the day of event and arrangements can be made.  Locker rooms are typically accessible at least two hours prior to scheduled start time for events.

Teams traveling without a certified athletic trainer are asked to:
1.  Notify the athletic training office in advance
2.  Provide a properly-stocked medical kit
3.  Include a letter (or email) with all names of athletes requiring treatment as detailed as possible (parameters of any modality treatments)

Athletic Training Room Policies
Athletic training staff cell phones:
1.  All athletic training staff members are equipped with a cell phone for EMERGENCY USE ONLY!
2.  Athletes MAY NOT reach a member of the athletic training staff outside of scheduled office hours without exception!  Your call will not be answered.
 
Conduct:
1.  No personal cell phones are permitted in the athletic training room at any time
2.  Proper attire by all is expected since it is a co-ed facility
3.  Food is not permitted
4.  Athletic equipment is to be kept outside the athletic training room facility
5.  All rehabilitation is to be scheduled in advance
6.  Injuries sustained during practice/away game must be reported by noon the following day via email or office visit
7.  Foul language or inappropriate behavior or conversation will not be tolerated and may be cause for dismissal from athletic training room at the staff discretion
8.  All athletes must be clean and showered after practice/games prior to receiving treatments
9.  Any athlete requiring treatment in the athletic training room must be supervised by a certified athletic trainer, not a coach or other staff member
10.  Water and supplies must be returned to the athletic training room in a timely manner following practices and put away in the appropriate area
 
Evaluation and treatment of varsity athletic injuries are available during office hours; however, busy conditions may require in season athletes to have priority.
At no time are water coolers or ice chests to be taken from the athletic training room for any reason without prior consent from the head athletic trainer.

Athletic Team Medical Coverage
The following varsity teams will be provided on-site ATC coverage for all in-season and post-season competition:
1.  Men's and Women's Soccer
2.  Field Hockey
3.  Men's and Women's Volleyball
4.  Men's and Women's Basketball
5.  Men's and Women's Lacrosse
6.  Baseball
7.  Wrestling
8.  Men's and Women's Tennis
9.  Men's and Women's Fencing
 
Teams which practice and compete off-site (equestrian, golf, cross country/track) will be issued a medical kit for all first-aid needs.  Athletes medical information is also kept in medical kits should an emergency arise.  An ATC will be provided for all home competition events for the above sports.  Should any athlete for the previously-named sports require pre-game treatments, they are to see the athletic training staff prior to departure for their event.  All coaches for varsity athletics have their assigned ATC’s cell phone number if needed.  For all events off-site, protocol requires that 911 be utilized in case of emergency.
 
Varsity Sport Assignment
Each athletic trainer is responsible for providing daily on-site and/or off-site practice and event coverage for his/her primary team during that team's traditional (in-season) season.  The purpose of this design is to ensure the best quality of care for our student-athletes.  It is the responsibility of the student-athlete to report all medical issues to their primary athletic trainer when possible, although athletes may be seen by any member of the staff.  Reporting injuries/illness directly to your assigned ATC is preferred and can be done by visit or via email if after hours.

Please review the weekly schedule to better understand which athletic trainer is covering your particular practice and what times they are available.  The athletic trainers will be covering all home events for their particular sport unless otherwise posted.
 
Medical Kits
All in-season varsity sports are assigned a medical kit for the season.  It is fully stocked by our staff prior to the season with all necessary first aid supplies.
Any athlete requiring special medications (inhaler, Epi-pen, etc.) must carry this with them for all away events.  In addition, one must also be provided for the athletic training staff to keep in the office.
 
It is the responsibility of the coach to be sure their kit is brought to each away event and returned.  In addition, it is their responsibility to be sure the kit is restocked as needed.  Please return your medical kit promptly to the athletic training office at the conclusion of your season.  This is very important as we have limited kits available and need them for the next in-season sports.
